I-RISE Interest Form
Increasing Representation in Supervisor Education, or I-RISE, a program offered through the Center for Practice Transformation (CPT), was funded by the Minnesota Department of Health (MDH). The grant was created to increase the completion of training requirements necessary to become licensing supervisors by those who identify as a member of an underrepresented community. In other words, these grant funds were awarded to cover the cost of supervision training for those who identify as a person of color, LGBTQ+, and/or disabled and who agree to deliver a minimum of 25% of their annual client encounters to people receiving publicly funded insurance or a sliding scale fee discount. Eligible participants also needed to be fully licensed as an LICSW, LPCC, or LMFT.
CPT will apply for future grant opportunities issued by MDH to fund a similar program in the future. If funded, CPT will reopen our I-RISE application.
Instructions: Please complete this interest form in its entirety if you meet the eligibility criteria above, or will in the future, and are interested in receiving communication about the I-RISE program if funding becomes available again. Completion of this interest form does not guarantee an award of funding.
